Your First Visit
On your first visit to our office, Dr. Cacchiotti will examine you/your child’s teeth and bite to let you know all of the treatment options available. This visit is free of charge! Often at the same appointment we may recommend some radiographs and photos for a more complete evaluation. Again, there will be no charge for the records we take.
Once treatment has been recommended, our Treatment Coordinator will discuss in detail the recommendations, cost of treatment, and payment options. We will also confirm any insurance benefits at this time.

Patient Forms
Feel free to fill out and submit the online patient forms. If you are unable to fill them out online or download them, we will be happy to send them to you in the mail so they can be completed prior to the first visit. Otherwise, please come 15 minutes before the appointment time to complete them in our office.
